Why does GLP-1 medication start at a low dose?

GLP-1 medications usually start at a low dose because your body needs time to get used to the medication.

This is especially important with GLP-1s because increasing the dose too quickly can make side effects more likely.

The process of slowly increasing your dose over time is called titration.

Why titration matters?

The early doses are often not the dose you stay on long term. They are there to help your body adjust.

As treatment continues, the clinician will review how you are doing, including:

  • whether you are having side effects

  • how well you are tolerating the medication

  • whether your dose should increase, stay the same, or be adjusted

Do not change your dose unless the clinician has advised you to do so.

Wegovy injection titration

For Wegovy injections, the standard titration schedule is:

  • 0.25 mg weekly

  • 0.5 mg weekly

  • 1.0 mg weekly

  • 1.7 mg weekly

  • 2.4 mg weekly

Each step usually lasts 4 weeks, as appropriate.

Wegovy tablet titration

For Wegovy tablets, the standard titration schedule is:

  • 1.5 mg daily

  • 4 mg daily

  • 9 mg daily

  • 25 mg daily

Each step usually lasts 30 days, as appropriate.

Zepbound titration

For Zepbound, the standard titration schedule is:

  • 2.5 mg weekly

  • 5 mg weekly

  • 7.5 mg weekly

  • 10 mg weekly

  • 12.5 mg weekly

  • 15 mg weekly

Each step usually lasts 4 weeks, as appropriate.

How to discuss changing your dose

You will have an opportunity at your refill check-in to discuss your dose with the clinician. They will review your goals and any side effects you are experiencing, then decide the appropriate next step.
